Who We Are East Longmeadow Skilled Nursing Center is a 119-bed rehabilitation and skilled nursing facility. We’re set way back off the road on 14 acres of wooded land bordering the Franconia Golf Course. We’ve been caring for people in the East Longmeadow area since 1965, providing top-quality, personalized rehabilitation, nursing, and long-term care in a warm, homelike setting. We’re locally run and staffed by caring and attentive East Longmeadow-area healthcare professionals. And we’re backed by Berkshire Healthcare, one of the largest, nonprofit, long-term care providers in Massachusetts. From the North
Take I-91 South to Exit 1 (Longmeadow). This is Rte. 5/Longmeadow Street. Proceed south to the third traffic light. Turn left onto Converse St. Follow Converse St. for three miles until it ends at Dwight Rd. Across the street is our main entrance, marked by a green sign. For additional parking, turn right onto Dwight Rd. from Converse St. Proceed one block and turn left onto Maple St. Go an eighth of a mile and turn left into our Maple St. entrance.

From the South
Take I-91 North to Exit 49 (Longmeadow). Turn right onto Rte. 5/Longmeadow St. Proceed approximately three miles and turn right on Converse St. Follow Converse St. for three miles until it ends at Dwight Rd. Across the street is our main entrance, marked by a green sign. For additional parking, turn right onto Dwight Rd. from Converse St. Proceed one block and turn left onto Maple St. Go an eighth of a mile and turn left into our Maple St. entrance.

From East Longmeadow Rotary
Rte. 83 enters from both Springfield and Connecticut. Exit the rotary onto Maple St. and proceed about one mile. Turn right into the facility by the sign (back entrance). Parking lot is on your left. To reach the main entrance, continue on Maple St. to next light. Turn right onto Dwight Rd. Turn right into our main entrance.
